秦火與孔天逸同是民營〔慈佑綜合醫院〕外科第一主刀，秦火擅長腦腫瘤手術，孔天逸擅長胸腔手術，各領風騷！
醫院來了一批實習醫生，顧嵐自幼被父親顧修源醫生拋棄，母女相依為命，半工讀修完醫科，展開她超齡實習醫生的生涯。趙詩婷是位聰穎，反應特快的女孩，她喜歡炫耀自己，因此很容易傷害別人而不自知！
顧嵐等實習醫生早聞秦火大名，能夠追隨他，驚喜萬分！不過接觸下，竟然大失所望，秦火冷面鐵腕教導方式，令他們誠惶誠恐，怨聲載道。尤其是大大咧咧的顧嵐，秦火一直認為她沒天份當一名醫生，但顧嵐仍忍受重重壓力，刻苦努力。
一宗病案，秦火下了確診，但顧嵐卻抱有懷疑。可是詩婷認為顧嵐的懷疑在醫學上的比率是萬分之三，秦火認同詩婷的說法，顧嵐不得要領，此事被孔天逸知悉，天逸認為病人的生命不能延誤，顧嵐也覺得是，還主動協助天逸。而事後顧嵐慘被大家誤會倒戈，卻令秦火對顧嵐的態度大為改觀！
此時顧嵐在急診室極力爭取機會，孔天逸欣賞她的幹勁，不時給她提點和照顧，兩人因此擦出愛的火花…
秦火要進行為愛滋病患動手術，詩婷等感恐懼而卻步，顧嵐自告奮勇跟秦火進手術室，秦火被顧嵐的行為感動，自此對她另眼相看，顧嵐在秦火悉心指導下，她又在醫術上跨進一大步…。
秦火與顧嵐在工作中，感情培增，秦火在顧嵐的影響下，性格也顯得隨性和活潑！實習醫生們與秦火消除了昔日的籬笆，視他為大哥哥，尤如一家人，同心同力的和病魔搏鬥！
孔天逸最終悔悟，在顧嵐和秦火的幫助下，天逸終於戰勝了自己，和眾人奮鬥在生命的第一線…
正在皆大歡喜之際，醫院卻爆發了傳染病被徹底的封鎖，眾志成城。秦火，孔天逸抹去一切的恩怨與顧嵐等全醫院的醫護人員攜手為病人的生命創造美好明天…！
